Patient Referral

We are happy to see your patients for cosmetic and implant treatment and, additionally, we are also happy for you to use the services of our hygienist and the facility of our Fresh Breath Clinic. We hope you will be happy with the service we provide and the knowledge that your patients will always be returned to your care, unless you specifically request they remain patients at Cleveland CDIC. If your patient requires implant treatment, you will be invited to accompany them to any of their appointments should they feel more comfortable with a familiar face nearby or you would like to view any of the procedures.

How it works

  • Dentist to send fully completed referral form (on line, by post or by fax).
  • Cleveland CDIC will send written confirmation of receipt of referral form.
  • Our Smile Brochure is sent to your patient.
  • Your patient will be contacted one week later to make an appointment- there will be a fee of *£130.00 for the consultation, radiographs and detailed examination.
  • At the consultation and detailed examination appointment, radiographs will be taken and a thorough examination carried out, a treatment plan will be discussed and rough ideas on cost will be given.
  • A report on the consultation and detailed examination will be sent to the referring dentist.
  • Should your patient require a detailed quote, which is necessary for treatment to proceed, there will be a fee of £100.00 which is refunded should treatment go ahead.
  • You are welcome to attend any appointments or implant operations, should your patient request this.
  • You will be advised of the completion of your patient’s treatment and any necessary after care they require.
  • Patients who have undergone dental implant treatment will be requested to attend Cleveland CDIC for yearly or two yearly routine examinations (fee payable) and may be requested to see our hygienist.

*Should your patient be uncertain about undertaking Dental Implant or Cosmetic treatment, we offer a FREE consultation, without the detailed examination, to give them an idea of what would be possible. Your patient will still be in the practice for approximately 1 hour and will be seen by both a treatment co-ordinator and a dentist.

From time to time at CDIC we have run courses for dentists here.

  • An introduction to Dental Implants – one and a half day course including lectures, hands on training and live patient demonstrations.
  • Nobel Active – an advanced half day course to introduce Nobel Biocare’s new implant system to those  practitioners already using Nobel Replace dental implants. Lectures, hands on training and live surgery. (PDF)

Lunch & Learns

We hold monthly lunch and learn sessions for dental practices to inform dentists and their staff on the basics of dental implants and update them on current trends: these sessions attract 1.5 hours of CPD. Cleveland CDIC will provide lunch for your whole team. If you would like to arrange a lunch and learn session within your practice please contact us and we will be happy to make the arrangements for you.
